As a new patient at CTNC you will be required to fill out multiple forms. This will include a New Patient Packet, in addition to the appropriate Health History Questionnaire. The New Patient Packet includes a welcome letter, demographics, authorizations and our HIPAA privacy policy. There are various Health History Questionnaires specific to the problem for which you are seeing us and are designed to obtain a detailed medical history.
All forms can be printed at home, filled out and brought to the visit. There is no online submission for forms. If you are unable to print forms on the website we are able to provide them for you here -- Please take fill-out time into consideration when planning your arrival for the visit. We kindly ask our patients to adhere to the arrival times listed for each doctor in their form links provided below. We understand the wait for your visit can be bothersome -- this protocol allows ample time for our staff to enter your medical history, medications and insurance information into our electronic medical record before your appointment. Failure to comply to these visit guidelines may result in longer wait time for your visit and, in some instances, the need to reschedule your appointment.
